Well Matt, the difference is purely psychological. Defensio offers me the possibility (and does advertise it prominently) to only register for an account to delete my Wordpress spam. This option is not as obvious on Wordpress.com. Maybe you should write "e.g. for an Akismet API key" behind your "Just a username, please." option on Wordpress.com or simply put another sign-up button on the front page to "Get an Akismet API key!". I did not decide for Defensio because I thought that Akismet wouldn't be able to do the job. I've actually heard of Akismet first and it seemed to be the right solution.
